Also since IANA has no more IPv4 addresses left you can see sometimes different people sharing the same IP when they are on the same ISP. That why we don't IP ban.

For ban evading it is not that easy to make a new account. You need a hack tool for that. If that user could evade bans he would not really care the got banned I assume.

Anyway it was more likely a server restart and not restarting of your PC fixed it.

In CoD4 1.0 to 1.7 it gets calculated from the cdkey as well. Method: MD5 over the 1st 16 lowercase characters. But MD5 uses a custom initialization vektor.
