In most cases such a loop is caused by the map / mod files being different in the server directory and the www-download directory. Make sure that is not the case...

Just a hint on how to safely modify it with hexeditor: find all "cod4master" strings and change the value of the first character in each of them to 0x00.

Try running the server with '+set dedicated 1'.

I assume windows 7? Right click the 'speaker' icon on your task bar, select recording devices, right click on the list and select 'show disabled devices', find stereo mix and enable it.
